Background
The service user is a retired teacher, mother of four and grandmother, who had always lived an active and independent life. She was also the main support for her husband, who is blind.
Following a stroke that reduced her mobility and increased her personal care needs, she spent over six months in a nursing home. Although well cared for, her greatest wish was to return home and live safely and comfortably in familiar surroundings.
Care needs at a glance

- Nutritional needs are fully met, with enjoyment taken in cooking alongside carers
- She is now happily living at home with her husband, maintaining their life together
- Family visits take place in a relaxed, homely environment
- The care package has remained stable and positive for nine months, with ongoing improvements in wellbeing and independence
